Position Overview:
The Care Coordinator Supervisor at You First Health Systems will oversee and manage a team of Care Coordinators to ensure the delivery of high-quality case management services to clients. The Supervisor will provide leadership, support, and training to Care Coordinators, ensuring compliance with organizational policies and state regulations. This role requires strong leadership skills, a deep understanding of mental health services, and a commitment to improving client outcomes.
Key Responsibilities:
- Supervision and Management:
- Oversee daily activities of the Care Coordinators to ensure effective and efficient service delivery.
- Provide regular supervision, guidance, and support to Care Coordinators.
- Conduct performance evaluations and provide constructive feedback to team members.
- Ensure that Care Coordinators maintain accurate and up-to-date client records.
- Monitor caseloads and workloads to ensure balanced distribution and quality care.
- Training and Development:
- Develop and implement training programs for new and existing Care Coordinators.
- Stay informed about best practices and emerging trends in care coordination and mental health services.
- Provide ongoing professional development opportunities for staff.
- Ensure Care Coordinators meet the Department’s training and certification requirements.
- Quality Assurance and Compliance:
- Ensure compliance with all relevant laws, regulations, and organizational policies.
- Conduct regular audits of client records and care plans to ensure quality and accuracy.
- Address any issues or discrepancies identified during audits promptly.
- Ensure that all documentation and reporting requirements are met.
- Client and Family Support:
- Address complex cases and provide support to Care Coordinators in managing challenging situations.
- Ensure clients and their families receive appropriate care and resources.
- Facilitate resolution of client complaints and concerns.
- Monitor client satisfaction and outcomes to identify areas for improvement.
- Collaboration and Communication:
- Foster a collaborative environment within the team and with other departments.
- Liaise with healthcare providers, community organizations, and other stakeholders to coordinate services.
- Participate in multidisciplinary team meetings and case conferences as needed.
- Communicate effectively with clients, families, and service providers to ensure continuity of care.
- Program Development and Evaluation:
- Assist in the development and implementation of care coordination programs and initiatives.
- Evaluate program effectiveness and make recommendations for improvements.
- Assist with grant writing and reporting as needed.
- Participate in strategic planning and organizational development activities.
Qualifications:
- Master’s degree in Social Work, Psychology, Counseling, or a related field.
- Licensed Mental Health Clinician: Licensed Clinical Social Worker (LCSW), Licensed Clinical Professional Counselor (LCPC), or equivalent credential preferred.
- Minimum of 1 year of experience in mental health case management or care coordination, with at least 1 year in a supervisory role.
- Strong understanding of mental health conditions, treatment modalities, and community resources.
- Experience in conducting Child and Adolescent Needs (CAN) assessments is a plus.
- Excellent leadership, communication, and organizational skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in electronic health records (EHR) and other relevant software.
